This is an alcohol and drug assessment used for probation agents, diversion agreements, or any
other legal situations.
This assessment is also used by individuals concerned about their own usage.
Assessment Description
This is a 1-1 meeting with the counselor. This is a two-hour appointment. After a history is taken,
collateral individuals are contacted and recommendations are made.
Cost/Payment
Cost: $175.00
You need to pay $100.00 before you can schedule this type of appointment. The remaining $75.00 is
due the day of your appointment.

*payment must be made before meeting with counselor
